Integrated Team Care Outreach Worker Chronic Disease

Level: 5-6

Salary: 78,185 - $85,282 per annum/pro rata

Hours/Employment Status: 37.5hrs per week

The Chronic Disease Integrated Team Care Outreach program is part of the Community Safety & Social Services Division, which aims to improve the quality of life of Central Australian Aboriginal people and the wider community through providing innovative, responsive and culturally appropriate services.

This program is funded by the Northern Territory Primary Health Network (PHN) specifically as a strategy to tackle Chronic Disease through improving the access of Aboriginal Australians to specialists, general practitioners and allied health services. The successful applicant will work in collaboration with another health service within Tangentyere Council; Tangentyere Aged & Community Services (TACS).

Tangentyere Council Aboriginal Corporation is seeking an Aboriginal Health Worker, Enrolled Nurse or experienced Outreach Worker to join our team.

This position requires a satisfactory Police Check and Ochre Card.
